I found this one when tidying up as well. It is a gentle song for Eucharist by Michael Mangan with a singable tune and plenty of verses.
The lyrics are on page 16 of this document from Litmus Productions. The sheet music is available in the the collection “Setting Hearts on Fire”, which can be purchased at Litmus.

I met the delightful Michael Herry briefly at the APMN conference. He distributed this song, the text of which speaks for itself.
